• Go to www.utsystem.edu/hcqrm in any Internet browser
• Click on the "Quiz and Text " button on the left
• The Instruction page will come up- Scroll down the instruction page
• Click where it says: "Click Here to Begin Course Insite Text & Quiz"
• The Log in Page will come up- All physicians log in as STUDENTS for the Risk Management Quiz

USER NAME & Password: ALWAYS your Social Security # without hyphens/spaces

• The Welcome page will come up
• Click on the words "Medical Legal Survival Course"
• Table of Contents should then come up
• Click on each module to read it
• When you have finished reading the text
• Click on the "Take a Test" tab or the word "TEST"
• The Test Instructions page should come up
• Scroll down and again put in your Social Security numbers for your user name and your password
• Click on the button at the bottom that says "Take Test" --This will put you into the test.
• 50 multiple-choice questions- Select and click on the correct answer
• Never use the browser "Back" button on your computer- This will cause you to restart your test from the beginning.

At the end of the test, if you left any questions marked "unsure" you will be asked to choose an answer. You will be shown a list of test questions and your answers. To change an answer, select the button next to that answer and click "Change Answer To Selected Question." When you satisfied with your answers, CLICK "FINALIZE" at the bottom of the page.

DO NOT TRY TO PRINT THE TEST OR THE ANSWERS – IT WILL CAUSE YOUR SYSTEM TO CRASH AND YOU WILL HAVE TO RE-TAKE THE QUIZ.

Follow the prompts & scroll down and you will see a "Certificate of Completion" button-Please print out your certificate when you pass the quiz and take it to your department administrator. Sign it and make a copy for your own records.

If you are in the middle of taking the quiz and must leave, but you do not want to start over…

• Click on “EXIT” at the top right of the screen.
• This will take you out of the quiz- call if you have a problem! 512/499-4511
• Then you may shut down your computer as usual.
• When you are ready to continue with the Risk Management quiz, you must log in again. FOLLOW THE SAME STEPS TO LOG ON/TAKE A TEST.
• This will put you back into the last question you answered, prior to exiting the quiz.

If you want to make extra copies of the certificate or your score at a later date, follow these instructions:

• Log in Click on the course name "Medical Legal Survival Course"
• Click on "Passed" and print your score and credits
• Click on "Certificate of Completion" and print your certificate
