This patient was a 40 year old male with chest pain and difficulty breathing. He denied prior cardiac disease, diabetes, hypertension, or illicit drug use. HR 100. BP 180/95. Temp. 103. His breath smelled of ethanol and ketones. His pupils were dilated. The cardiac and pulmonary exam was unremarkable. His urine was dark. An EKG and serial cardiac enzymes were ordered. Does this patient have an acute myocardial infarction? Answer
